Family Information Form for Group Babysitting

This ‘Family Information Form’ is used for additional families included in the one Booking and gathers vital information on the family and the care of the children to assist the Nannies on the day.

After a Booking has been made with the Agency, additional family‘s included in the booking will need to complete this form that includes : — routine, dietary restrictions, medical conditions, special requirements and a COVID declaration for the family.
